Instructions for Applications/Evaluation/Reference Forms
Please read all of the following information below.

All forms can be faxed to (716)773-1748.
If fax appears to not be transmitting please call us at the same number.

*If you desire to coach for the Niagara Power
please call Cal Kern first at (716)773-1748.

Players and Coaches:

Please read the following information before sending in any form to the Niagara Power:

To Play with the Niagara Power:

► You must currently be a full-time college/university student, having completed a minimum of one semester (12 credit hours) at that school.

► You cannot be a graduating senior.

Change Since Last Year Due to Change In Coaching:
► Junior College players CAN play for the Niagara Power this season. (NCAA rules state that if a summer collegiate team has a Division I, II, or III coach they cannot coach over junior collegiate players.)

We need from each applying player: ►Player Application Form

►Player Evaluation Form from a current baseball coach

►3 Reference Forms -

• 1 from a personal contact (cannot be a family member)

• 1 from a coach/mentor (past or present)

• 1 from a minister

We need from each coach
that wants to coach for the Power: ►Coach Application Form

►3 Reference Forms -

• 1 from a personal contact (cannot be a family member)

• 1 Professional

• 1 Pastoral

If you are the coach of
an applying player: ►Fax a player evaluation form to the above number

We need from each applying intern: ►Intern Application Form

►3 Reference Forms -

• 1 from a personal contact (cannot be a family member)

• 1 Professional

• 1 Pastoral
